
A woman has facial aging issues and has visited 3 different surgeons, but all of them have different opinions. She wants to know which of those procedures are appropriate for her.
Dr. Amiya Prasad, a specialist in eyelid surgery and facial rejuvenation surgery, explains that all surgeons have a certain style in how they do things and the own way they look at things, which are why cosmetic surgery is not a generic. One of the most common mistakes people make is thinking that an upper or lower eyelid lift is the same no matter who they go to. As a specialist, he sees patients from all over the world who come to him in his offices in Manhattan for many different types of complications or undesirable results. For some people, their eyelids are pulled down, their face was not lifted well enough, and some have undesirable scarring.
Dr. Prasad explains that one of the things that you could’ve done when she visited doctors was to review their results and look at people that may share some resemblance to her in terms of ethnicity, age, and anatomy. Experienced surgeons have lots of examples of their results. Dr. Prasad has been in practice for over 20 years so he has a lot of pictures of his patients’ before and afters. This lets patients look through and helps them understand his style.
Dr. Prasad agrees that her upper eyelids have hooding, her lower eyelids have puffiness, there are wrinkles around the mouth and there is some volume loss in the cheek area and sagging of the face and neck. Routinely in his practice, he would address the upper lids with an upper eyelid blepharoplasty, the lower eyelids with a lower eyelid blepharoplasty, typically a transconjunctival approach. Sometimes, people with puffiness who are over 40-years-old will often have laxity of the lateral canthal tendon and that needs to be reinforced. For the skin quality around the eyes, he routinely does platelet-rich plasma (PRP)that is derived from a patient’s own blood that improves skin quality through collagen and blood vessel generation. In addition, he would do fractional CO2 laser to address wrinkles around the eyes and mouth.
In patients who want to have a lot of volume in the face, Dr. Prasad places submalar implants or cheek implants. It may be invasive, but when someone is dealing with a lot of sagging, there’s only so much volume they can put in the skin before it sags as well. As far as the skin, face, and sagging are concerned, face and neck lifting can be done in a variety of ways. Ultimately, her goal is to restore jawline, elevate the cheek, and have a nice angularity of the neck and just an overall younger appearance.
Without doing a physical examination, Dr. Prasad has some agreements with the colleagues who made their recommendations and he has his own personal style on how he addresses a lot of these issues. Every doctor will do things that they feel works for them. Procedures are not generic. In one colleague’s hands, dermabrasion may work as well as laser in another colleague’s hands. Each will have their reasons for the benefit of that treatment. It comes down to her being comfortable with whoever it is that will do her procedure. If she remains confused, then she must get additional opinions. It’s very important that she is not conflicted when she’s having your surgery. If she doesn’t have trust and complete faith in her doctor before she has surgery, then it’s going to be a rocky road after that.
Whenever patients come to Dr. Prasad and say that he is one of the doctor’s they’re meeting, if there is something that sounds different from what he is recommending, they are welcome to ask him about it. He will certainly share with them why he agrees or disagrees. A lot of doctors have a tendency to be disrespectful to their colleagues which is very unprofessional. Everyone is smart and has experience. At some point, Dr. Prasad suggests that she make a decision when she’s comfortable and have some faith to move forward with her surgery.
